A completed health form is required for each participant to attend.This form is kept only for medical emergencies and therefore we encourage you to include with it a list of all current medications and a copy of your health insurance card(s) (front and back). This form can be kept in a sealed envelope, if you desire, and returned to you at the end of the retreat. The waiver however, must be turned in at the beginning of the event.
