Program Overview
The purpose of the Cardiovascular Institute of Philadelphia Inaugural Interventional Cardiology Fellows Program is to provide fellows with an interest in interventional cardiovascular medicine with a practical, focused, in depth experience of left ventricular support and state of the art approaches to complex coronary interventions.  The program features nationally recognized experts leading small grouup learning sessions; case reviews and hands on simulations will be utilized as educational tools.  For interventional fellows fulfilling requirements, certification in the use of the Impella axial flow pump and rotational atherectomy will be offered. 

Case Competition and Award:  Left Ventricular Support with the Impella Axial Flow Pump

*Use PowerPoint format - 10 slides maximum including cine loops and videos.  Presentations using more than 10 slides will be disqualified.
*All submitted cases will be reviewed by the CVI Interventional Cardiovascular Medicine Education Committee
*Winner will be informed via email by February 24, 2017
*Winner will present the case to our faculty and attendees from 1:20pm-1:35pm at CVI Inaugural Interventional Cardiology Fellows Program and receive an Apple iPad.

*Confirmed attendees of CVI's Inaugural Interventional Cardiology Fellows Program are eligible to receive (1) one complimentary standard rom at the Loews Philadelphia Hotel for Saturday, March 4, 2017
*Rooms are provided so that attendees of CVI's Inaugural Interventional Cardiology Fellows Program can stay over to attend CVI's 5th Annual Interventional Cardiovascular Medicine CME program on Sunday, March 5, also at the Loews Philadelphia Hotel
*Sleeping rooms will be available after 4:00pm, following the Loews Philadelphia Hotel's normal check-in policy.  Sleeping Rooms must be vacated following the Hotel's normal check-out policy, before 11:00am
*CVI is underwriting the cost of standard room rates and applicable taxes
*All other expenses including but not limited to parking, room service, restaurant charges etc are the responswibility of the attendee
*Attendees will be required to produce a valid driver's license or other form of identification as well as a major credit card at check-in to cover any incidental charges described above
*Cancellation policy - If you are unable to attend the program, you must cancel your reservation directly with CVI no later than 4:00pm on Wednesday, March 1, 2017 by calling 215-389-2300 or emailing cvi.janetnoel@verizon.net

TRAINING CERTIFICATIONS FOR ROTATIONAL ATHERECTOMY AND LV SUPPORT
Please note:  Certification opportunities are offered to Interventional Cardiology Fellows.  If additional spots are available and time allows, opportunities will be provided for other attendees.
PRE-REQUISITES AND REQUIREMENTS TO RECEIVE CERTIFICATIONS 
LEFT VENTRICULAR SUPPORT WITH IMPELLA AXIAL FLOW PUMP
In advance of the program, attendees wishing to receive Impella Certification must complete the following:

*Register and review through www.abiomedtraining.com
   - A certificate of completion will be generated at the conclusion of the Abiomed online training.  The certificate must be sent via email to cvi.janetnoel@verizon.net or via fax to 215-389-5450 by Friday, February 24, 2017 to schedule your simulation/wet model training.

*Must successfully complete wet model/simulation training at CVI Fellows Program

Within 3 months of the program (by Wednesday June 3, 2017) you must complete 2 Impella cases as lead operator and provide attestation by an attending physician to CVI in order to receive your Impella Training Certificate.  If you have already completed >  Impella cases by that time as lead operator, you can submit it in advance directly to CVI, or please bring your attestation letter with you to the CVI Fellows course.

ROTATIONAL ATHERECTOMY WITH THE ROTABLATOR SYSTEM
In advance of the programn, attendees wishing to receive Rotablator System Certification must complete the following:

*Register and complete Boston Scientific's Online Rotablator Course at the following lin: http://www.bostonscientific.com/en-US/products/plaque-modification/rotablator-rotational-atherectomy-system/training.html
   -A certificate of completion will be generated at the conclusion of the Boston Scientific Rotablator online training.  The certificate must be sent via email to cvi.janetnoel@verizon.net or via fax to 215-389-5450 by Friday, February 24, 2017 to schedul your simulation/wet model training.

*Must successfully complete wet model/simulation training at CVI Fellows Program

Within 3 months of the program (by Wednesday, June 3, 2017) you must complete 3 patient cases utilizing Rotational Atherectomy with the Rotablator system as lead operator and provide attestation by an attending physician to CVI in order to receive your Rotablator Training Certificate.  If you have already completed >3 Rotablator cases by that time as lead operator, you can submit it in advance directly to CVI, or please bring your attestation letter with you to the CVI Fellows Course.
