Jax Physique, competitors have the option to fill out/sign your entry form and mail it with a money order, or, you have the option to pay your entry fee online with a credit card. Please read the following instructions first.

Registration Deadline is April 1st, 2017. If postmarked after Saturday April 1, 2017, Late Entry will be $90.00 for each division entered.

Online deadline to register is Wednesday April 5th, 2017. 

Register in person at the Check-Ins and Weigh-Ins on Friday April 7th, 2017.

ONLINE PAYMENT INSTRUCTIONS: If you are paying online with a credit card please be sure to follow these instructions:

1. Fill out the 2017 Jax Physique Entry Form.
2. Scan and email to jaxphysique@aol.com.
3. Be sure to select the correct Entry Fee option!
4. You must bring a copy of your completed online registration information to check-ins.
