Transformative Healthcare in 2017: The Physician-led Model 

About the Program: The expectations regarding health care quality, efficiency and patient safety are constantly evolving. HealtheParadigm offers a suite of services to help the healthcare professional adapt to the ever-changing technological landscape.

The HealtheParadigm goal is to improve health care quality, coordination and efficiency through the exchange of health information at the point of care, utilizing a secure electronic network that ultimately streamlines a patient’s  information.

The physician-led health information network in Georgia is built upon a proven technology model which enables the sharing of clinical information at the point of care, while supporting physicians’ efforts to meet the new Quality Payment Program MIPS or APM requirements through the use of analytics. 

Providing actionable intelligence to providers is crucial to clinical integration and the transition to the Quality Payment Program models. Having the right information at the right time will enhance patient care, support quality initiatives and lower malpractice risk.

Plan to join Cardelia Reid, RN, MSN, MBA, and regional director for HealtheParadigm, as she describes physician-led efforts to utilize and aggregate clinical data to improve patient care. She also will explain the infrastructure that connects EHR systems, and demonstrate data analytics for clinical reporting.
