Improving Care for Older Adults through Interprofessional Education (Afternoon session) 

Interprofessional care is essential to quality care of older adults. Often times, older adults face multiple complex problems that require assessment and intervention from various healthcare professionals. By learning how to incorporate perspectives across multiple health and social service disciplines, we enhance problem solving to develop a comprehensive and integrated care plan, inclusive of the older adult, their caregivers, their values and goals. Healthcare professionals will be encouraged and supported by the input and collaboration from other professionals. This session will focus on a real life example of an older adult who has suffered from a traumatic health care event.

Objectives:

  • Identify signs and symptoms of depression and grief. Discuss how these may contribute to functional decline in an older adult.
  •  Develop an appreciation of the importance of geriatric syndromes on function and quality of life in older adults.
  •  Explain why older patients are more vulnerable to medication adverse effects
  •  Identify medication issues in  elderly patients that may contribute to functional decline
  •  Discuss the challenges of optimizing/maintaining independence and safety in the care of older adults.
  •  Describe the importance of family/healthcare advocates in the care of older adults.
  •  Establish effective communication strategies that acknowledge the patient family/healthcare advocates as part of the interprofessional team.
  •  Verbalize the unique contributions team members play to create a more complete picture of the clinical, psychosocial, and spiritual aspects impacting health.
  •  Discuss the TeamSTEPPS approach and utilize selected TeamSTEPPs tools.

This program is FREE and open to the public. 2.0 continuing education credits are available to social workers and nurses.

  • This program is approved for two continuing education units for LSWs, LISWs, SWAs, RNs (reciprocity), pharmacists, and physicians

Physicians should claim only the credit commensurate with the extent of their participation in the activity. Summa Health System is accredited by the Ohio State Medical Association to provide continuing medical education for physicians.
