Welcome to Homeless Health Care Los Angeles' Care Coordination and Systems Navigation Training!


Homeless Health Care Los Angeles is excited to announce no cost, regional training opportunities in collaboration with United Way that are available throughout Los Angeles County.

Course title: Care Coordination and Systems Navigation

Course Description: Individuals who complete this program will be able to demonstrate essential skills of homeless services Care Coordinator by mastering the necessary skills to effectively assess navigate and connect people experiencing homelessness to housing and supportive services.


This five day, 35-hour course will instruct the following topics:

 

Day 1: Landscape of Homelessness in Los Angeles County

  • Introduction to Homeless Services
  • Overview of Homeless Services sector in Los Angeles County
  • Risk Factors and Demographics of Homelessness in Los Angeles County
  • Homeless Definitions and sub-populations
  • Case management Core functions
  • Street Outreach

 

Day 2: Core functions of Homeless Case Management

  • Open ended questions
  • Assessment
  • Case Planning
  • Documentation


Day 3: Morning :Boundaries, Ethics and Cultural Awareness 

  • Boundaries and Introduction to HIPAA
  • Cultural Identification
  • Humility vs. Competency
  • Stereotypes and Discrimination
  • Person Centered Language

 

Day 3: Afternoon: Housing and Benefits

  • Types of Housing
  • Rental Assistance
  • Qualifications Coordinated Entry Process
  • Housing Programs
  • "Document Ready"

Day 4: Effective Practices

  • Trauma Informed Care
  • Stages of Change
  • Harm Reduction
  • Housing First
  • Motivational Interviewing

 

Day 5: Health, Thriving in the Community & Navigating the System

  • Health Care Barriers and common chronic diseases
  • Holding difficult conversations
  • Behavioral health "Severe & Persistent" Diagnoses, Treatment Options
  • Substance Use  Commonly Used Drugs and Their Effects,  Treatment Options, Dual Diagnosis

 

Course Measurement:

This course is measured through a pre and post-test, and participant demonstration of group activities and exercises that reinforce the learning objectives of each module.

 

Pre-requisites: None


Completion: A certificate of completion and a 35 hours credit of title will be awarded to participants that complete the course.


Who should attend: Homeless services staff who have worked at their agency between 0-3 months are being prioritized for this year of no cost training.
