HRST WEBINAR 10-20-17 and 10-25-17

Phase 3 of the Health Risk Screening Tool (HRST) will be implemented beginning Mon., Nov. 6, 2017.  The remainder of individuals receiving services in the Community Pathways Waiver are to be screened.  Those who have already been screened will not need a new HRST.  Screening for individuals in both Residential and Day Habilitation will be completed.

Coordinators of community services will continue to be responsible for performing screening of the HRST.
