Educational Objectives:
At the end of this lecture, the learner should be able to do each of the following:
Make a logical case for addressing medical, behavioral, and social complexities in an integrated approach.
List at least four of the five levels of integration required to achieve optimal health outcomes in a primary care patient panel.
Apply the same strategies for a “bundled-care” approach to major surgical procedures (e.g. joint replacement surgery) or hospital admissions (acute coronary events).
Identify at least one key step that they could take to begin building or strengthening multi-level integration in their own practice or patient panel.
