Continuing Medical Education Credit

These activities have been planned and implemented in accordance with the accreditation requirements and policies of the Accreditation Council for Continuing Medical Education (ACCME) through the joint providership of PeerPoint Medical Education Institute and the Home Centered Care Institute.  PeerPoint Medical Education Institute is accreditated by the ACCME to provide continuing medical education for physicians.

PeerPoint Medical Education Institute designates these activities for a maximum of 15 AMA PRA Category 1 Credits.  Physicians should claim only the credit commensurate with the extent of their participation in the activity.

Essential Elements of Home-Based Primary Care 

HCCI is committed to training the future HBPC workforce, bring more providers and practice managers into the field, and set clinical and operational standards.

HCCI Essential Elements of Home-Based Primary Care Workshop was developed in collaboration with experts in the fields of HBPC, adult education, and instructional design. This 2-day workshop explores the full spectrum of home-based primary care - medical, social, and economic. HCCI's curriculum offers a strong foundation in HBPC and is relevent to home-based primary care practitioners and practice managers, but also for any professional who provides or supports patient care in the home.

Part of this registration includes three 1-hour Webinars (monthly, beginning the first month following the Workshop. Webinars included are Creating a Business Plan and Budget for a Home-Based Primary Care Practice, Staffing Models and Strategies for Home-Based Primary Care, and Building Value and Metrics in Home-Based Primary Care.

Course enrollment also provides access to an exclusive HCCI University online discussion forum which includes downloadable tools, forms, and templates. This shared learning experience offers the opportunity for our experts to address your questions and challenges, following your Workshop attendance.
