Patient Engagement: Activation & Driving Outcomes

Learning Objectives:

  • Review Site Self Assessment (SSA) data and PTN TCPi Aims with a focus on patient engagement.
  • Explain the role of Occupational Therapists in integrated healthcare settings and the approach OT in treating the whole patient beyond the exam room.
  • Discuss the importance of engaging patients in decision making on the personal, administrative, and system levels of health care.
  • Review measures submitted by practices and inform actions which can be taken towards quality improvement.
