HCCI Advanced Applications in Home-Based Primary Care builds on the Essential Elements workshop and is designed for clinical providers and practice managers/operations staff with some experience in providing HBPC services and who are seeking to advance their knowledge, skills, and productivity to provide this type of care for patients with complex medical conditions. This workshop is also ideal for those who have completed the HCCI Essential Elements of Home-Based Primary Care workshop and are ready to continue their learning.
HCCI's curriculum features case-based sessions focusing on advance care planning, communication, and prognostication; care coordination and transitions of care; caregiver support; managing challenging clinical issues including dementia, polypharmacy, acute/urgent care and wound care; coding, documentation and billing for services beyond E/M; staffing models, workflow/scheduling, and productivity; and value-based partnerships.
Workshop enrollment also provides access to an exclusive HCCI University online discussion forum and supplemental workshop information. This shared learning experience offers the opportunity for our experts to address your questions and challenges following your Workshop attendance.
