TapRoot®: Near Misses and Root Cause Analysis Training


Learn to analyze accidents, incidents, near-misses, equipment failures, operating issues, or quality problems using TapRooT® techniques. The 5-Step investigation process simplifies TapRooT® to make the investigation efficient yet effective. Training focuses on understanding what happened, identifying Causal Factors, analyzing the Causal Factors using the Root Cause Tree®, and developing effective fixes using the Corrective Action Helper® Module. You will get the chance to use the TapRooT® System to analyze an actual incident brought by you or one of your team members.

Instructor:

Steve Swarthout is the owner of Process Improvement International, Inc. and is a Certified TapRoot Trainer.  With 35 years of equipment failure analysis under his belt, Steve brings a wealth of knowledge to accident investigations.  He has worked in commercial nuclear power and was a member in the US Navy.  His major clients include Kellogg, Ameren and Shell Oil, among others.
