The fourth in this series profiles the innovative INSPIRED COPD Outreach Program™; a High Impact Practice developed in Nova Scotia.

This palliative approach to care enables patients and families to better manage advanced COPD at home, decrease the sense of isolation and distress, and reduce dependency on the emergency department and inpatient services.

Learn from a Certified Spiritual Care Practitioner and Social Worker about how their roles as Advance Care Planning Facilitators at Nova Scotia Health Authority address unmet needs of patients and families, whilst creating significant efficiencies.

PRESENTERS

DARCY GILLIS
Darcy is a CASC Certified Spiritual Care Practitioner with the Nova Scotia Health Authority in Halifax.

Darcy started working as an Advance Care Planning Facilitator and Research Coordinator with the INSPIRED COPD Outreach Program in 2014. After the completion of the research project on the feasibility of expanding the INSPIRED program into an emergency department setting, she became a full time ACPF with the INSPIRED program in 2016.

Darcy also sits as the Regional Ethics Chair on the Canadian Association of Spiritual Care’s National Ethics Commission and is passionate about patient- and family centered care, and the way that advance care planning further integrates respect for autonomy and the person as a whole into end-of-life care.

ANDREW COMSTOCK
Andrew is a registered Social Worker with the Nova Scotia Association os Social Workers and is employed by Nova Scotia Health Authority.

Andrew began working as an Advance Care Planning Facilitator with the INSPIRED COPD Outreach Program in 2018. Prior to this role he worked as the Hospital social worker for the Palliative Care Program at the Queen Elizabeth II Hospital in Halifax from 2011 to 2015, and then on the Medical Teaching Unit from 2015-2018.

Andrew also works casually as a Crisis Work at the IWK Children’s Health Centre in Halifax. His passions include counselling and providing advocacy for persons nearing end of life, trauma and grief work.

Both Darcy and Andrew provide psycho/social and spiritual support, as well as facilitate advance care planning discussions and the completion of personal directives, with INSPIRED patients and family members.

JEANNE BANK
Jeanne is a Project Lead with the Canadian Home Care Association. She is currently the project specialist for the CHCA initiative, Building Operational Excellence in Home-Based Palliative Care.

Her expertise is in the areas of policy and standards development, stakeholder engagement, and association management. She is a member of several iternational standards committees in the areas of vulnerable persons, aging issues, and community resilience.

Jeanne also holds a certificate in Public Involvement throuh the International Association of Public Participation (IAP2).
