ADVANCING OPERATIONAL EXCELLENCE IN HOME-BASED PALLIATIVE CARE

INTEGRATING A PALLIATIVE APPROACH TO CARE BY HAVING CONVERSATIONS EARLY (IPACE)
Empowering and mentoring frontline staff to incorporate early conversations with patients living with a serious illness about what matters most so that their wishes are respected across health care settings.

Vancouver Coastal Health's IPACE program provides training to frontline staff across disciplines and care settings to help them routinely identify which patients living with serious illnesses can benefit from a palliative approach to care. The training encompasses how to have conversations about serious illness with patients so that their wishes, values, and goals can be documented so that these follow patients across health care settings. The program supports frontline staff to find solutions to integrate the IPACE work in their daily practice. Outcome measures are shared with leadership and frontline staff so that they are aware of how these early conversations impact patient quality care.

PRESENTERS

INGRID SEE
Ingrid is the Clinical Lead for the Nancy Chan Palliative Care Ambulatory Clinic (NCPCAC) and the Vancouver Community Lead in the IPACE project with Vancouver Coastal Health.

Ingrid has a background in home care nursing and has worked extensively as a Clinical Nurse Specialist in palliative care for many years in the community, acute, and residential care settings. Her role is to improve clinical knowledge and standards of practice in palliative care and also provide specialized nursing consultation to patients and families coming to NCPCAC.

Ingrid has a Masters in Education which focused on developing education materials specifically around cultural competency and end of life care.

SARAH LAU
Sarah is the Clinical Educator for IPACE in Vancouver Community with Vancouver Coastal Health.

Sarah has over ten years of Home and Community experience and developed a passion for palliative care when working in acute care and the community. She finds it a huge privilege to walk with people at such an intimate juncture in people’s lives.

She is responsible for educating, mentoring and supporting change in community areas to consider a palliative approach early.

JEANNE BANK
Jeanne is a Project Lead with the Canadian Home Care Association. She is currently the project specialist for the CHCA initiative, Building Operational Excellence in Home-Based Palliative Care.

Her expertise is in the areas of policy and standards development, stakeholder engagement, and association management. She is a member of several iternational standards committees in the areas of vulnerable persons, aging issues, and community resilience.

Jeanne holds a certificate in Public Involvement throuh the International Association of Public Participation (IAP2).
