HOME-BASED PALLIATIVE CARE

VIRTUAL PALLIATIVE CARE INTERVENTIONS
Championing virtual care to empower patients and build system capacity.

WEBINAR
Tuesday 24, September 2019
12:00 - 13:00 ET

The next webinar in the Canadian Home Care Association's series on operational excellence in home-based palliative care showcases an innovation in Ontario that integrates technology into clinical practice.

Learn from our speakers at William Osler Health System about the development and pilot of a remote self-reporting app for patients and 'Virtual Home Visits'. Hear how virtual care models could transform home-based palliative care, providing timely intervention while patients remain as independent and safe as possible in their homes.

COMPLIMENTARY REGISTRATION: This webinar is supported through the knowledge translation of the "Operational Excellence in Home-Based Palliative Care" project. The project has been made possible through the Health Canada Policy Contribution Program.
