2019 Team Up, Take Action: A Conference on Partnering to Improve Community Health 

Thank you for your interest in sponsoring the 2019 Team Up, Take Action: A Conference on Partnering to Improve Community Health. 

The New Hampshire Public Health Association is excited to once again co-present this conference with Dartmouth-Hitchcock Health and the Vermont Public Health Association! Approximately 150 public health, health care and community-based professionals attended this event in 2018, representing a wide variety of organizations and community sectors.

Now in its fourth year, the Team Up, Take Action conference convenes public health and health care professionals, community activists, and health advocates to advance best practices and build collective capacity to improve community health through partnership. The 2019 conference theme is Building Bridges between Communities and Clinical Environments. What makes for successful partnerships between clinical health care providers, public health systems, and community stakeholders? What are the essential strategic skills for leading partnerships? What are the tough challenges in integrating clinical and community systems that focus on disease prevention and improve health outcomes for all? What can we learn from others who are building partnerships in different settings? What policy changes can help to facilitate strong partnerships in the future? What practices and approaches to organizational development and collaboration can best promote inclusion of all?

Our keynote speaker will be Jose T. Montero, MD, MHCDS, Director, Center for State, Tribal, Local, and Territorial Support at the U.S. Centers for Disease Control and Prevention!

Once again, we will partner with academia to provide an opportunity for students to network and become more active in public health in New Hampshire. CEUs will also be offered.
