The Kentucky Statewide Diabetes Symposium committee wishes to acknowledge our
Industry Allies Council (IAC). Participation in the Council illustrates dedication to the professional community of diabetes educators and the state diabetes symposium. IAC provided through unrestricted monies.


To Be Announced

Industry Allies Council Benefits:

  • Pay one fee which includes exhibit fee, meal card, and continuing education credit for up to three (3) company representatives who are either a nurse, dietitian, or pharmacist.   
  • Recognition of level of support announced during the symposium with a special presentation during lunch.    
  • Recognition of level of support included in the participant packets (if received by 10-21-19).
  • Recognition of level of support noted on signage displayed prominently at the symposium.
  • Recognition of level of support through table displays.
  • Option to provide a meal or break (may not be the sole provider).

 Exhibitor Benefits:

  • Pay one fee which includes exhibit fee, meal card, and continuing education credit for up to two (2) company representatives who are either a nurse, dietitian, or pharmacist.  
  • Recognition as an Exhibitor included in attendee packet (if received by 10-21-19).

Allies Council (IAC) / Exhibitors:

Complete online registration as soon as possible (payment may occur later).

. Space is limited and will be allocated based upon completion of the online form on a first-come, first-serve basis.
Payment – IAC / Exhibitors may choose to pay online by credit card or by check. If payment must be by check: Make Checks Payable To:
Kentucky Diabetes Network (KDN) with notation for "Kentucky Statewide Diabetes Symposium 2019" and mail with "Online Registration Form" to: Kentucky Statewide Diabetes Symposium 2019, c/o Green River District Health Department, 1501 Breckenridge Street, Owensboro, KY 42303. KDN is a 501 (C) 3 organization.

If Continuing Education (CE) credit is desired, a separate PARTICIPANT REGISTRATION FORM must also be completed at .                                                                                   Obtaining Continuing Education (CE) CreditIAC / Exhibitors who are a nurse, dietitian, or pharmacist may obtain CE credit if the representative follows all CE attendance requirements (register online, attend the entire program, complete the evaluation form, etc.). CE space will NOT be reserved for IAC / Exhibitors unless they also register separately on the "Online Participant Registration Site" at  IAC / Exhibitors may register for CE on a first-come, first-serve basis. IAC may receive continuing education credit for up to 3 company representatives while Exhibitors may receive continuing education credit for up to 2 company representatives.
