SSCC World Diabetes Walk and Field Games 2019 

All students, faculty, and staff are invited to join the Shelton State Association of Nursing Students (SSANS) to unite for diabetes awareness during the World Diabetes Day Walk and Field Games on Thursday, November 14 from 12:00 p.m. - 4:00 p.m. Onsite registration will begin at 12:00 p.m. The opening ceremony will begin at 1:00 p.m.

Important Information

In order to participate in the World Diabetes Walk and Field Games, registrants must agree to the terms and conditions of the Diabetes Walk Waiver. Please review the Diabetes Walk Waiver before continuing. By clicking "I Agree," you confirm you have carefully read and reviewed the Diabetes Walk Waiver, and that you understand it and execute it voluntarily. 

Diabetes Walk Waiver

If you are under 18 years of age, a legal guardian must sign a waiver for you to particpate in the walk and games on the day of the event. 

By continuing to register for this event, you acknowledge agreement to each of the following statements: 

  • I am 18 years of age or older.
  • I voluntarily take part in the 2019 World Diabetes Day Walk and Field Games and the offered health assessment. 
  • I am physically fit and sufficiently trained to participate in the event.
  • I understand that the results for the health assessment performed are for information purposes only, and not for the diagnosis of any medical condition, as such results may not be accurate.
  • I agree that no doctor-patient or nurse-patient relationship is created as a result of today’s assessment.
  • I understand that Shelton State Community College is not a health care institution and therefore does not diagnose or treat any medical conditions.
  • I waive and release any and all rights and claims for damages I may have against the walk, sponsors, and their representatives for any and all injuries suffered by me in said event.
  • I agree to release Shelton State Community College from any damage or injury that may result from participation in the identified diabetes walk and health assessment/screening opportunity.
  • I grant full permission to use my name, as well as photographs and any record of this event in which I may appear for any legitimate purpose, including advertisement and promotion.
