Course Description:
Have you ever read a patient’s case notes and found them to be insufficient in detail, or that information has been omitted, or found a note was not completed for a procedure? Is there a lack of calibration in the way case notes are drafted among your dental team? Accurate documentation is imperative, yet the topic is rarely discussed. This informative course acknowledges the increase of dental hygienists’ clinical responsibilities, as well as, the importance of accurate documentation. We will evaluate the American Dental Hygiene Association’s Standards of Clinical Care, which provides the foundation and model for documentation’s key elements. We will demonstrate the benefit of templated notes to achieve consistency for all members of the dental team. Delving deeper into documentation, we will discuss informed consent, legality issues, referrals, treatment planning, scheduling, time management, and purposeful conversation as it pertains to the patient’s continuity of care, and we will examine the American Academy of Periodontology (AAP) Periodontal Classification update. This course is suited for dental hygienists, but valuable information is provided for the entire dental team.

Mary Ann Vazquez, RDH, BSDH
Mary Ann Vazquez BSDH, RDH is a registered dental hygienist presently employed at Hooksett Family Dental, Hooksett, New Hampshire. She achieved a Bachelors of Science degree in Dental Hygiene from Northern Arizona University and attained an Associates Degree in Applied Science from Delaware Technical and Community College. Using her education and clinical experience she has given presentations to the senior population on topics such as Xerostomia and Oral Health Considerations with Parkinson’s. Collaborting with pediatricans and local dentists, she organized Infant Oral Assessments as her capstone project, a topic she is passionate about. Prior to a career as a dental hygienist, Mary Ann proudly served in the United States Air Force as an Inventory Management/Logistics specialists. In her spare time Mary Ann enjoys watching New England sports teams, spending time at the beach and making memories with her family.
