Program Description:

LEAP Core is a two-day course that provides health care professionalswith an in-depth learning experience on essential skills andcompetencies of the palliative care approach.This course takes an inter-professional approach and promotesteamwork and collaboration among different health careprofessionals who work in a variety of settings and organizations.

 Who is it for?

LEAP Core is ideal for any health care professional (e.g. physicians,nurses, pharmacists, social workers, etc.) whose primary focus of work is not palliative care but who provides care for patients withlife-threatening and progressive life-limiting illnesses.

  • Two days, 14 hours in total
  • Face-to-face learning with group discussion and case studies

  • Course materials include a downloadable manual and thebest-selling resource, the Pallium Palliative Pocketbook

 

This Group Learning program has been certified by the College of Family Physicians of Canada for up to 28 Mainpro+ credits. For more information about Pallium Canada, please visit www.pallium.ca.

Requirements: The workshop materials are downloadable from the Pallium portal. Following the registration deadline, Pallium will create an account for you on the Pallium Portal. You will receive an email notification once this has been completed. Materials, pre- and post-course activities and workshop evaluations are available through the Pallium Portal. Please ensure that you bring a laptop/tablet or your own copy of the materials for your reference during the workshop. Hard-copies of the materials will not be available on-site. 

FEE: $950.00 + HST

This workshop is being hosted as a collaboration between the OCFP and Pallium Canada. 

CANCELLATION POLICY
The OCFP reserves the right to cancel sessions due to unforeseen circumstances or insufficient advance registration. In the event of a cancellation made by the OCFP, a full refund will be given to the registrants. However, the OCFP cannot accept responsibility for out-of-pocket expenses due to cancellation of a session. Cancellations made more than 15 business days prior to the date of the event will be refunded, less a 25% administration fee. No refunds will be provided after this deadline.
