Von's Vision Day Kansas City 

Von's Vision is providing a city-wide vision day for low-income children in Kansas City while the Broncos are in town to play the Chiefs. Each child receives a free eye screening and if needed, a free eye-exam and corrective eye wear. The eye screening and exam will occur on the day and time listed.

Before registering your child, please print off these forms and fill them out. Each child will need to have the Patient History Form and the Parental Consent Form completed before they can participate in Von's Vision Day. If you are not bringing your child to the program, please ensure they have a printed and signed copy of the attached forms before they leave for the program.
