SD -HRST reviews for initial plans
(Is the nurse an HRST Trained reviewer? If she is please ask if she can review the plan so we can process it. Please ensure the plan has nursing included as a requested service (it is ok if it is new money, Please ensure you request enough funds for this review and the next one so it becomes a part of the budget) Once the plan is approved and the funds allocated, the nurse should be able to receive payment.)

Nursing
Health Services/Regional RNs: HRST as risk mitigation and identification tool. Risks to be identified and mitigated in the PCP --risk and health and wellness area when health related Consider the RNCMDN NCP also has risks and mitigations that may be helpful when identifying in the PCP.
Issues: HRST reviews for folks in services but no nurse in plan: Identify nurse--Regional nurses have lists of current and active RNCMDNs for each county These nurses are raters and reviewers, however--there is no nursing support service identified in the waiver for HRST review only. If HRST review indicated, there are significant risks to address. If delegation needed then this is the nursing support service. If no delegation : consultative nursing support service or health case management..
