Advanced Care Planning (ACP), and communication in palliative and end-of-life care is essential but can be difficult for all involved. This training is designed to assist Interprofessional working in palliative care initiate and carry out conversations with patients and their families about goals of care and preferences at the time of admission, at regular intervals, and when there has been a decline in health status.
Specifically designed as an interdisciplinary training program for primary care physicians, nurse practitioners, physician assistants, and nursing staff.
This course will:
-Review communication principles associated with palliative and end-of-life care
-Demonstrate specific skills for initiating and engaging patient or family in managing and executing ACP
-Discuss the complications, risk factors, and appropriate documentation for specific procedures
