This volunteer day between Revitalize Milwaukee and the State Bar of Wisconsin Construction and Public Contract Law Section is instrumental to our mission to keep low-income Milwaukee families safe, one home at a time.

Victoria is 34 and lives in the River Bend neighborhood with her two children, Jazlyn and Jetzen. Both children have disabilities and love to play outside, but they live right next to a drainage ditch that they frequently try to get into and a main road with cars speeding by. Their current fence is too short and does not go around the entire yard which leads to the children climbing over it to get to the drainage ditch and sometimes strangers passing through her yard to drink and do drugs near the drainage ditch. These issues are big safety risks to Victoria and her children.

By installing a higher and sturdier fence around the entire yard, Victoria will have peace of mind letting her children play outside and her children will be safe from the hazards of the drainage ditch, the road, and the strangers that used to pass through her yard.

Lunch will be provided and there will be drinks at the end. If you have your own tools you are welcome to bring them.

General Volunteer Publicity/Liability Waiver and Release

In consideration of being permitted to participate as a volunteer with Revitalize Milwaukee on November 13th, 2021 (known as “Activity”), an event that is intended to keep a local family safe and build community morale, and in light of the aims and purposes of the community service provided by Revitalize Milwaukee, I, on behalf of myself and my assigns, heirs, and next of kin:

  1. Agree that my participation in the Activity is wholly voluntary and that I am fully aware of the inherent hazards, dangers, and numerous risks involved in working at a construction site where projects are currently under construction, including serious injury and/or death. I HEREBY ASSUME FULL RESPONSIBILITY FOR ANY RISK OF BODILY INJURY, DEATH ARISING FROM OR IN CONNECTION WITH THE ACTIVITY, OR DAMAGE TO MY PROPERTY, INCLUDING INJURY OR DAMAGE CAUSED BY ANY PRODUCTS OR EQUIPMENT USED AT OR IN CONNECTION WITH THE ACTIVITY.
  2. Release and forever waive Revitalize Milwaukee and any of its administrators, directors, agents, officers, members, volunteers and employees, other participants, any sponsors or advertisers, and owners and lessors of the premises on which the Activity takes place (known as “Releasees”) in connection with my participation in the Activity from any and all claims or obligations against the Releasees, both known or unknown, both present and future, that arise from, during, or because of my participation in the Activity caused by my own account, and I further agree that if, despite this Waiver and Release I, or anyone on my behalf, makes a claim against any of the Releasees, I will indemnify, save and hold harmless each of the Releasees from any litigation expenses, reasonable attorney fees, loss, liability, damage or cost which may be incurred as the result of such claim. Injury, illness, death or damage resulting from intentional or reckless conduct are not released by this Waiver
  3. Agree to permit and authorize Revitalize Milwaukee and its employees agents and representatives who are acting on its behalf to use, edit, alter, copy, exhibit, publish, or broadcast my likeness and/or name in any photograph, image, video, motion picture, performance, or sound recording from the Activity for any purposes including advertising, publicizing, or marketing its business and products, or for any other commercial or lawful purpose, and to use and license others to use it for such purposes without any compensation to me.
  4. HAVE READ THIS WAIVER AND RELEASE, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT AND HAVE SIGNED IT FREELY AND WITHOUT INDUCEMENT OR ASSURANCE OF ANY NATURE AND INTEND IT TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY THE LAW AND AGREE THAT IF ANY PORTION OF THIS AGREEMENT IS HELD TO BE INVALID, THE BALANCE, NOTWITHSTANDING, SHALL CONTINUE IN FULL FORCE AND EFFECT.

General Volunteer COVID-19 Liability Waiver and Release

The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people.

Revitalize Milwaukee has put in place preventative measures to reduce the spread of COVID-19; however, Revitalize Milwaukee cannot prevent you or other individuals from becoming exposed to, contracting, or spreading COVID-19 during the volunteer event on November 13th, 2021 (known as "Activity"). Therefore, if you choose to participate in the Activity, you may be exposing yourself and/or increasing the risk of contracting or spreading COVID-19.

By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I may be exposed to or infected by COVID-19 by attending the Activity and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 from the Activity may result from the actions, omissions, or negligence of myself and others, including, but not limited to, Revitalize Milwaukee employees, volunteers, and program participants and their families. I consent to any reasonable requirements established by Revitalize Milwaukee to ensure the health and safety of you and other people during the Activity including, without limitation, to have your temperature taken or wear a protective mask. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to myself (including, but not limited to, personal injury, disability, or death), illness, damage, loss, claim, liability, or expense, of any kind, that I may experience or incur in connection with my attendance at the Activity (known as "Claims"). On behalf of me and my spouse, domestic partner, heirs, assigns, guardians, and legal representatives, I hereby release, covenant not to sue, discharge, and hold harmless Revitalize Milwaukee, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, reasonable attorney fees, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of Revitalize Milwaukee, its employees, agents, volunteers, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any Revitalize Milwaukee event. Injury, illness, death or damage resulting from intentional or reckless conduct are not released by this Waiver.

I agree that if I test positive for COVID-19 within 14 days after the Activity ends, I will notify Revitalize Milwaukee of such test results within 3 days after receiving the test results.
