Rosemary Gerstner, Marketing and Sales Director at Senior Helpers of Greater Chicago, will discuss the effect of micro-social determinants of health on aging in place and how to reduce your risk for hospital readmissions.

WHAT ARE SOCIAL DETERMINANTS OF HEALTH?

Social determinants of health are the economic and social conditions that influence population differences in health status. Micro-social determinants of health are the factors that impact an individual as opposed to an entire population, e.g. their home environment.

Our conversation will focus on the following micro-social determinants of health:

• Independence (ability to perform daily activities)
• Burden of Care (access to resources from family, caregivers, and the community)
• Safety Risks (assessing areas of concern around the home)
• Quality of Life (enjoyment of social interaction and leisure activities)
• Medical Conditional Management (monitoring medical conditions and medication).
