If you are registering as a TEAM (4 players):
- The individual filling out this form will be considered the "team captain" and will be the primary contact for additional information
- Add your team members information (including handicaps) using this form *if you do not know your team members now, you must email their information to BIAV no later than Monday, October 5, 2022*
If you are registering as an INDIVIDUAL, you may skip the "team member information" section of the form.
