Von's Vision provides a Vision Day program to eligible children who need eyecare or corrective eyewear. Each student receives a free eye screening and if needed, a free eye-exam and corrective eye wear. The eye screening and exam will occur on the day and time listed.

Before registering on behalf of your child, please print off these forms and fill them out. Each student will need to have the Patient History Form and the Parental Consent Form completed before they can participate in Von's Vision Day. Please bring all eyeglass prescriptions to your appointment, if child currently wears glasses. If you are not bringing your child to the program, please ensure they have a printed and signed copy of the attached forms before they leave for the program.
