Seminar: “Stronger Together: How Legacy Lifecare Collaborates to Accelerate Improvement in Nursing Homes” Senior care leaders today face an unprecedented level of challenges to provide high quality care while maintaining financial viability. During this interactive 2-part session, participants will learn the collaborative framework that Legacy Lifecare uses to support nursing homes in continuously improving performance. This framework has allowed us to critically analyze how we do our work, find new ways to collaborate both internally and externally, and adapt to a rapidly changing environment. In the second part of the session, we will directly address one of the major sources of concern for nursing homes – how to achieve and maintain an overall CMS 5-star rating. Lastly, specific tips and guidelines will be shared on how to engage different stakeholders to tackle problems and accelerate improvement.

Speakers: Ron Anglo, PhDc, RN, MSN, MBA, RAC-CT is the Chief Clinical Officer at Legacy Lifecare and a Registered Nurse with 25+ years of clinical experience. Katherine Santos, MBA is the Chief Strategy Officer at Legacy Lifecare and oversees the organizational strategies for advancing clinical and operational excellence which includes the formation, tracking, communication, and achievement of strategic goals.

Seminar: "International Dysphagia Diet Standardization Initiative (IDDSI) Engagement for Team Leaders: Promoting the Standard of Practice" The International Dysphagia Diet Standardization Initiative, IDDSI, has earned recognition across the United States and throughout the world as “THE” standard of care for those who benefit from texture modified food/liquids. Ten years in development, IDDSI prioritizes safety for all cultures, settings, and ages, improving communication and patient outcomes during transitions of care. Additional benefits of IDDSI implementation can include optimized quality of care with improved resident satisfaction and reduced unplanned weight loss. With increased attention to measurement & consistent food production, waste can be minimized, and staff efficiency improved, with possible benefits including improved expense control. Across the world, as well as within local communities, people speaking the same language of IDDSI are providing quality care and support to individuals with swallowing problems (dysphagia).

Speakers: Mary Rybicki, MS, RD, LDN is a Registered Dietitian Nutritionist with diverse clinical and management experience in post-acute care settings. Katrina Woodward, MS, CCC-SLP, CDP is a speech pathologist and Chief Administrative Officer of New England’s mobile Fiberoptic Endoscopic Evaluation of Swallowing (FEES) provider, SDX Dysphagia Experts.

Seminar:Value-based Payment Models & Long-term Care Resident Populations” Skilled nursing facility and assisted living facility operators have the opportunity today to participate in multiple value-based payment models for their long-term resident populations.  Speakers will discuss how these models work and how skilled nursing facility and assisted living facility operators may consider participating.  The models discussed will include: Medicare Shared Savings Program ACOs, ACO REACH (ACO Realizing Equity, Access, and Community Health), ACO REACH - High Needs, and I-SNPs (Institutional Special Needs Plans. 

Speaker: Chris Wasel, President, Marketing & Strategic Partnerships for Vantage Healthcare the largest physician practice of its type in New England providing services to skilled nursing facilities and assisted living facilities.

Seminar: "UNH Panel on Long-Term Care Workforce Development and Educational Institution Partnership" Panel of educators in the University of New Hampshire’s College of Health and Human Services who study various aspects of workforce development. Each panelist will present current topics of specific interest to the role of the university in preparing the future long-term care workforce.

Speakers: Corazzini: role of the public research university in healthcare workforce development partnerships. Bonica: The school-to-work transition and emerging adulthood – how to address the challenges of hiring and retaining young people leaving school (high school or college) and entering the workforce while navigating the tasks of emerging adulthood. Braswell: Discuss the benefits of partnering with a baccalaureate nursing program from the facilities and program’s perspectives. Highlighting the complexity of the needs of the residents provides rich opportunities for learning assessment, management of complex health problems, and working within an interdisciplinary team. Harkless: Integrating learners from all levels of nursing and other professions into LTC is needed to prepare the future workforce. Models for this learning integration are varied and include, among others, the Teaching Nursing Home, and the Dedicated Education Unit models. What are these models? How do they work in LTC?
