Palliative Care ECHO Hub Project Introduction

In response to the changing times, the CHCA is pleased to announce the launch of our Palliative Care ECHO Hub. Based on the principles of the “Project ECHO” model, the CHCA ECHO Hub will connect home care providers with palliative care specialists to address gaps and challenges in home-based palliative care.
