Let's talk about Advanced Health Care Planning
This educational offering will define Advance Health Care Planning and the importance of Advance Health Care Directives. Social workers will be available to assist in completion of HCPOA and Living Will documents at no charge. There will be two workshops - one from 1 to 3 pm in the afternoon and an evening workshop from 5 to 7 pm on the same day
