Vine After School Registration

We invite you to register your child(ren) for our Vine After School program beginning Wednesday, September 4th. This form must be completed and signed by a parent or legal guardian before a child (anyone under eighteen years of age) may participate in any regular church program, special outing, or other children/youth activity at FBC Mena.
