Medication Management:
Improving positive outcomes &
reducing adverse events
Health and economic impacts of medication related problems include polypharmacy, age related physiological changes and how to individualize drug therapy. Preventing medication related problems in older adults will be highlighted in this presentation. CMS and other national guidelines for appropriate use of medications will also be shared.
Presenter: Jag Khatter, PharmD, CDE, CGP
Jag Khatter is currently the V.P. of Operations for Pharmca reUSA Western
Region. He received his Pharmacy degree
from the University of Colorado. Jag has
received many awards including “Pharmacist
of the Year” in the state of Colorado. He is
board certified in Pain Management and
Diabetes. His clinical work involves
population health and managing geriatric
patients. Jag launched several diabetes
initiatives as a VP of Innovation for
Walgreens to increase revenues and establish Walgreens as the destination for people with diabetes.
Jag developed and executed a strategic plan for the growth of a “startup” Assisted Living Pharmacy (SeniorMed) where several new hub pharmacies were started to service assisted living communities in 30 states, using a “hub and spoke “model. Jag has also served as member of the Board of Directors for Joint Commission for Pharmacist Care Accreditation. He has served as Chairman of the Assisted Living Task Force for ASCP (American Society of Consultant Pharmacists) and Chairman for CCGP (Commission for Certification in Geriatric Pharmacy). Currently, he serves as a member of the Assisted Living Committee and as a member of Psychotropic Medications Task Force for CHCA
