SUPPORTING DISEASE SELF-MANAGEMENT:  TECHNIQUES TO INTEGRATE INTO EVERYDAY PRACTICE

We are pleased to present this quarter's topics and keynote speakers:  

THE "SPIRIT" OF MOTIVATIONAL INTERVIEWING:  BEST PRACTICES TO SUPPORT SELF-MANAGEMENT

Damara Gutnick, MD, is the medical director of the Montefiore Hudson Valley Collaborative (MHVC) where she is responsible for guiding systems and practice transformation efforts to support the transition to value based payment across the Hudson Valley as part of New York State’s Delivery System Redesign Incentive Payment (DSRIP) program.  She is an Associate Professor of Epidemiology and Population Health at the Einstein School of Medicine, an adjunct Associate Professor of Medicine and Psychiatry at NYU and Director of Quality and Research at the non-profit Centre for Collaboration, Motivation & Innovation.

An internist by training, Dr. Gutnick spent 18 years caring for patients at Bellevue Hospital, NYC’s largest public hospital and working for NYC Health and Hospitals Corporation, where she was committed to providing culturally competent, quality care to a diverse inner city patient population, and spearheaded behavioral health integration and self-management support initiatives. 

As a trainer of Motivational Interviewing (MI) and a member of MINT (The Motivational Interviewing Network of Trainers), Dr. Gutnick is passionate about encouraging clinicians to incorporate the “spirit” of MI into their discussions with patients and dedicated to integrating MI based best practices into health care delivery systems to improve patient engagement in their care.   With this aim in mind, Dr. Gutnick has consulted for various national healthcare systems interested in incorporating MI based principles into practice, and presented MI based workshops at national and international forums including the Institute of Healthcare Improvement, American College of Physicians, the Society of General Internal Medicine, and the MINT Forum. Dr. Gutnick is also part of the team who developed Brief Action Planning (BAP), a highly structured, person centered, stepped care, evidence informed, self-management support tool based on the principles and practice of MI.   Lastly, in her role at MHVC, Dr. Gutnick championed a Motivational Interviewing and BAP Train the Trainer program that developed 24 MI and BAP trainers within MHVC network partner organizations in the Hudson Valley.  The Train the Trainer program will empower MHVC partners with the ability to train staff in MI and other person-centered communication techniques beyond the DSRIP program. 

THE NYS SMOKERS' QUITLINE:  MOTIVATING TOBACCO USERS TO BECOME AND STAY TOBACCO FREE

Patricia Bax, RN, MS, TTS, CCis the Marketing and Outreach Coordinator for the New York State Smokers’ Quitline and Roswell Park Cessation Services.  In her role, Patricia has presented at statewide and national conferences and she has co-authored several papers.  

She holds a Bachelor’s Degree from the Niagara University School of Nursing, and earned her Master’s Degree in Counselor Education at Canisius College in Buffalo, New York.  Patricia is a Registered Professional Nurse and a Tobacco Treatment Specialist.   Throughout her professional career, Patricia has worked in Human Services and as an adjunct instructor at Niagara County Community College.  She hosted a behavioral health radio show and was a featured presenter for the National Wellness Conference.  She developed a four-session humor therapy program (SMILES) focused on developing a humorous perspective in aging and provides consultation services related to humor and resiliency.

USING EDUCATION TO EMPOWER PATIENTS TO MANAGE CHRONIC DISEASES

The Better Choices, Better Health® Workshop Series, also known as the Chronic Disease Self-Management Program (CDSMP) and the Diabetes Self-Management Program (DSMP), were developed by Stanford University. The evidence-based programs have been proven to help adults better manage their chronic conditions, improve their quality of life and lower health care costs.

Jessica Schreck, RD, CDN, from the Cornell Cooperative Extension will illustrate valuable benefits the diabetes program provides to patients and share experiences as a workshop peer leader.

Leslie Vicale, MPH, Project Manager of Clinical Improvement Strategy at the Suffolk Care Collaborative, will describe how the SCC is working with local partners to bring these programs to patients and how to refer to local workshops.

The Project Advisory Committee’s (PAC) mission is to promote collaboration and support the goals, strategies, and initiatives among the SCC partners with respect to improving the health status of Medicaid and uninsured communities in Suffolk County.
