Quality Consortium of Suffolk County
On Friday, October 27th, from 8:30am-12:30pm at the Coram Fire Department!
The landscape of our healthcare system is changing. As we move away from a fee-for-service environment and towards value based payment, we would like to extend this educational and networking opportunity for providers in Suffolk County to prepare for this new environment.
This mini-conference will bring together primary care providers, behavioral health providers, and hospital providers to constructively address identified barriers related to patient referrals to treatment.
Join us to:
- Recognize service availability in geographical regions/enclaves
- Build capacity to determine appropriate level of care
- Workshop methods to streamline the patient referral process
