Join HealthInsight's  Quality Improvement team, Regional Extension Center staff, and your community colleagues for an interactive event focused on applying patient centered care best practices in care transitions, cardiac health, and health information technology (HIT). Come and bring your care team!

 

We will start the day with a morning plenary followed by Learning and Action Network breakout sessions.

Morning plenary topics include:

  • How new payment paradigms, such as the accountable care model, create a rationale and business case for a shift to more patient centered care. 
  • How patient engagement strategies can prepare providers and patients to succeed in this new environment. 
  • Hear from a patient with kidney disease who successfully transformed his own care and health outcomes through active participation in his care plan.  
  • An overview of transitions of care community coaching. 
  • The status of HIT adoption in your community. 

Breakout sessions:

 

After the plenary, join your peers in smaller group discussions on advancing patient engagement in sessions focused on cardiac health, care transitions, and health information technology.
