"Engaging Patients and Families: On the Cusp of a New Frontier in Health Care"

Rachel Willard-Grace will share her knowledge on the importance of patient and family engagement, teach participants how to assess where their practice is now in terms of patient engagement and help practices identify strategies to better engage patients as partners in their care.

"Positive Partnerships for Hope" 

Jodi Hansen and her son, Jacob, will share their experiences and explain, from a patient perspective, why patient-centered care is so important in a physician office setting.

  • The importance of patient and family engagement in patient-centered medical homes;
  • How to assess where practices are in terms of patient engagement; 
  • Two strategies to better engage patients as partners; and

Breakout sessions:

After the plenary, join your peers in smaller group discussions on navigating transformation in sessions focused on patient and family engagment, chronic care managment, quality reporting & meaningful use and using care management codes.
