8:30am – 4:50pm: Health systems interventions, social determinants of health and evidence based programs for chronic disease prevention and management

Friday, June 10, 2016: All participants welcome!

8:30am – 4:50pm: Wellness initiatives, treatment guidelines and perspectives from the healthcare team

Objectives: 

  • Make connections between patient health and population health
  • Increase implementation of quality improvement processes in health systems through the use of the Electronic Health Record (EHR)
  • Improve use of team-based health care in health systems
  • Promote chronic disease self-management
  • Review screening recommendations, treatment guidelines and advances in technologies
  • Relate social determinants of health in regards to practice

 Who should attend: 

  • Health systems leaders, administrators and partners
  • Multidisciplinary health care providers
  • Quality improvement specialists, IT, medical office staff and those involved with the EHR
  • Public health professionals
  • Breast and Cervical Cancer Early Detection Program (BCC) clinical providers and staff (medical assistants, billing and coding specialists)
