Putting a Fall Prevention Program into Action

About this event: This session will utilize the QAPI approach for implementing or enhancing a fall prevention program, helping you to organize steps for implementation. The session will also provide you with an action plan template to move your program forward.

Objectives:

  • Identify performance gaps in current falls-prevention program to develop an action plan
  • Develop an interdisciplinary team approach and have effective team meetings
  • Demonstrate how to set benchmarking data and document a mobility enhancement and restorative nursing programs to meet Medicare/MDS requirements.

Presenter profile: Jeri Ann Lundgren, RN, BSN, PHN, CWS, CWCN has worked in long-term care since 1990. Over the past two years, she has focused on the effects of immobility and worked with exercise physiologists, physical therapists, personal trainers, ergonomics and nursing to develop mobility enhancement programs. Her extensive national background includes survey management, litigation support, wound-care consultation, program development, and she is a clinical expert for several state Quality Improvement Organizations and departments of health.

Ms. Lundgren’s background also includes four years as a director of nurses in a long-term care facility. She is currently president of Senior Providers Resource.
