Please join us for the EQRO Transition of Care Listening Session. The purpose of the meeting is to provide more eduation, direction and specifics to help with the Transition Planning process. 

 Please forward this invite to the Care Coordinators who are responsible for transition planning. 

We request that you bring no more than 10 people in person. We encourage additional participants to join us remotely through our Zoom Video Conferencing that will allow us to show documents to remote users. 

The documents and questions for the ToC Session are being developed and will be sent via e-mail in advance so that you can review them and prepare questions or comments. Please register to confirm your attendance by May 17, 2017.
