New Mexico Population Health Collaborative
Peer Learning Workshop

Don’t miss this opportunity for health systems and partners to come together to learn from each other and share experiences about using quality improvement processes and team-based care to improve patient outcomes for hypertension and diabetes management.

Participants will get an overview of the data collected from this project’s assessment and hear from local health systems involved in this project about what quality improvement efforts could look like in their own health systems, as well as their successes, lessons learned, barriers and next steps. Participants will also be able to evaluate opportunities to improve screening and management of chronic disease using health information technology.

This workshop is open for health systems that are participating in the New Mexico Population Health Collaborative.

Travel Reimbursements

  • Mileage or car rental – one vehicle per location. If you are traveling up to 80 miles from your location, we will reimburse for mileage. If you are traveling more than 80 miles from yur location, we will reimburse for vehicle rental.
  • Hotel for one night
