This release form should be completed by parents/guardians for every preschooler/elementary child in your family who plans on participating in the Living Word Discipleship Ministry - Junior Youth for Christ, retreats, camps, Milestones, cross-gen events, worship, and all other activities with Living Word Lutheran Church.
