Masjid Uthman Prayer Registration

Due to COVID-19 and CDC social distancing guidelines for the health and safety of community members and attendees, all attendees of daily and Jumuah prayers at Masjid Uthman must register using this form. As seats for this registration are currently limited, we request those to register who will pray regularly (daily) at Masjid Uthman.
