  • RELEASE AND CONSENT. IT IS ACKNOWLEDGED AND AGREED THAT THE EVENT (THE “EVENT”) THAT IS DESCRIBED IN THE DOCUMENTS, WEB PAGES, AND/OR ELECTRONIC COMMUNICATIONS ASSOCIATED WITH THIS RELEASE AND CONSENT IS A POTENTIALLY HAZARDOUS ACTIVITY AND I, FOR MYSELF, AND FOR ANY AND ALL OF MY MINOR CHILDREN OR PERSONS OVER WHOM I HAVE GUARDIANSHIP PARTICIPATING IN OR ATTENDING THE EVENT, AND ALL OUR HEIRS, ADMINISTRATORS, AND EXECUTORS (COLLECTIVELY, “PARTICIPANTS”), HEREBY VOLUNTARILY ASSUME FULL AND COMPLETE RESPONSIBILITY FOR, AND THE RISK OF, ANY INJURY OR ACCIDENT THAT MAY OCCUR DURING PARTICIPANTS’ PARTICIPATION IN THE EVENT.

  • TO THE FULLEST EXTENT OF THE LAW, PARTICIPANTS HEREBY RELEASE, HOLD HARMLESS, AND COVENANT NOT TO FILE SUIT AGAINST (I) THE MORTON CHAMBER OF COMMERCE AND ITS AFFILIATES AND THEIR RESPECTIVE DIRECTORS, OFFICERS, VOLUNTEERS, AGENTS, AND EMPLOYEES (COLLECTIVELY, “CHAMBER”); (II) ANY EVENT SPONSORS; AND (III) ALL OTHER PERSONS OR ENTITIES ASSOCIATED WITH THE EVENT (COLLECTIVELY, THE “RELEASEES”), FOR ANY INJURY OR DAMAGES PARTICIPANTS MIGHT SUFFER IN CONNECTION WITH THE EVENT. THIS RELEASE APPLIES TO ANY AND ALL LOSSES, LIABILITIES, OR CLAIMS PARTICIPANTS MAY HAVE ARISING OUT OF THE EVENT, INCLUDING BUT NOT LIMITED TO, PERSONAL INJURY OR DAMAGE SUFFERED BY PARTICIPANTS OR OTHERS, WHETHER SUCH LOSSES, LIABILITIES, OR CLAIMS BE CAUSED BY CONTACT WITH AND/OR THE ACTIONS OF OTHER PARTICIPANTS, CONDITIONS OF THE EVENT AND EVENT PREMISES, NEGLIGENCE OF THE RELEASEES, RISKS NOT KNOWN TO PARTICIPANTS OR NOT REASONABLY FORESEEABLE AT THIS TIME, OR OTHERWISE.

  • THIS RELEASE IS INTENDED TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW. In addition, Participants give consent and permission to the Chamber, its affiliates and sponsors, and their successors, licensees, and assigns, the irrevocable right to use, for any purpose whatsoever and without compensation, (i) any photographs, videotapes, audiotapes, or other recordings of Participants that are made during the course of the Event; and (ii) the results of Participants’ participation in the Event. Participants understand that (i) this Release and Consent is given in consideration for being permitted to participate in the Event; (ii) that Participants may be removed from the Event if Participants do not follow all the rules of the Event; and (iii) Participants are voluntary participants in the Event. Participants are in good physical condition and are solely responsible for their personal health, safety, and personal property.

  • In addition, Participants acknowledges that novel coronavirus (''COVID-19") infections have been confirmed throughout the United States, including several cases in Participants’ own State and locality. In accordance with the most recent guidance and recommendations issued by the World Health Organization, the Centers for Disease Control and Prevention (CDC), Participants’ own State’s Departments of Health for slowing the transmission of COVID-19, Participants hereby agree, represent, and warrant that they shall not visit or utilize the facilities, services, and/or programs of the Chamber (other than any exclusively online services and programs) within 14 days after (i) returning from highly impacted areas subject to a CDC Level 3 Travel Health Notice, (ii) exposure to any person returning from areas subject to a CDC Level 3 Travel Health Notice, or (iii) exposure to any person who has a suspected or confirmed case of COVID-19.

  • Participants agree to check on a daily basis the CDC Travel Health Notices list (https://www.cdc.gov/coronavirus/2019-ncov/travelers/index.html) prior to participating in or utilizing the Chamber’s facilities, services, and programs. Participant hereby agree, represent, and warrant that they shall not participate in, visit or utilize the Chamber’s facilities, services, and/or programs if they (i) experience symptoms of COVID-19, including, without limitation, fever, cough, loss of sense of taste or smell, or shortness of breath, or (ii) have a suspected or diagnosed/confirmed case of COVID-19. Participants agree to notify the Chamber immediately if they believe that any of the foregoing access/use restrictions may apply.

  • The Chamber has taken steps to implement certain recommended guidance and recommendations issued by public health agencies for slowing the transmission of COVID-19, including, without limitation, the access/use restrictions set forth above. Participants acknowledge and agree that the Chamber may revise its procedures at any time based on updated recommended guidance issued by public health agencies and further agree to comply with the Chamber’s revised procedures prior to utilizing and participating in the facilities, services, and programs of the Chamber. Participants further acknowledge and agree that, due to the nature of the facilities, services, and programs offered by the Chamber, social distancing of 6 feet per person among fellow Participants or others is not always possible.

  • Participants fully understand and appreciate both the known and potential dangers of participating in the programs and/or utilizing the facilities and services of the Chamber and acknowledge that use thereof by Participants, despite the Chamber’s reasonable efforts to mitigate such dangers, result in exposure to COVID-19, which could result in quarantine requirements, serious illness, disability, and/or death

  • This Release and Consent shall be construed under the laws of the State of Illinois. In the event any provision of this Release and Consent is deemed unenforceable by law, (i) the Chamber shall have the right to modify such provision to the extent necessary to be deemed enforceable; and (ii) all other provisions of this Release and Consent shall remain in full force and effect. Participants understand that Participants have given up substantial rights by signing this Release and Consent, and agree to these terms freely and voluntarily without any inducement, assurance, or guarantee.
