COVID 19, LOST PROPERTY, MEDICAL AND PHOTO WAIVER & RELEASE. Parents agree to keep their child home if they are sick. Camp takes no responsibility for Covid 19 issues, reserves the right to make changes due to Covid 19 and will give NO REFUNDS due to Covid 19. Every precaution will be taken against loss of personal property, but Camp OnAqua (SFWSC Inc.) will not assume responsibility in case of loss. Participation in camp indicates permission to use promotional photos and video of campers. Accident insurance is not available to campers through Camp OnAqua or SFWSC Inc. The responsibility for the cost of medical care is assumed entirely by the camper, parents or guardian. In case of an emergency, every effort is made to contact the parents or guardian. If parents/guardian are unavailable, I give permission to the physician selected by the Camp Director to hospitalize and secure proper treatment for my child. I accept responsibility for medical treatment charges, which may be incurred on my child's behalf.

RELEASE OF LIABILITY FOR MINOR PARTICIPANTS
READ BEFORE AGREEING
IN CONSIDERATION OF my child, being allowed to participate in any way in Camp OnAqua and related events and activities, I myself and my spouse, acknowledges and agrees that the risk of injury to my child from the activities involved is significant, including the potential for permanent disability and death. I MYSELF, SPOUSE, AND CHILD, KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES and assume full responsibility for my child's participation; and If I observe any unusual significant concern in my child's readiness for participation and/or in the program itself, I will remove my child from participation and bring such attention to Camp OnAqua immediately.
I myself, my spouse, my child, and on behalf of my/our heirs and or assigns HEREBY RELEASE AND HOLD HARMLESS ALL "RELEASEES" (Camp OnAqua, Catfish Bay, SFWSC Inc., its directors, officers, officials, agents, employees, volunteers, camp staff, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the camp} ("Releasees"), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property incident to my child's involvement or participation in Camp OnAqua, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law. I, for myself, my spouse, my child, and on behalf of my/our heirs, assigns, personal representatives and next of kin, HEREBY INDEMNIFY AND HOLD HARMLESS all the above Releasees from any and all liabilities incident to my involvement or participation in these programs, EVEN IF ARISING FROM THEIR NEGLIGENCE, to the fullest extent permitted by law
I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY AGREEING TO IT, AND AGREE TO IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT. I ACKNOWLEDGE THAT THERE IS A COVID 19 PANDEMIC. IF MY CHILD IS SICK WITH ANY COVID 19 SYMPTOMS I WILL KEEP MY CHILD AT HOME AND INFORM CAMP IMMEDIATELY. I WAIVE ANY CLAIM AGAINST ANY AND ALL “RELEASEES” IF MY CHILD CONTRACTS COVID 19 AT CAMP ONAQUA.
