• The child named above has my permission to participate in a Greg Guiler Basketball Event (Greg Guiler Basketball, LLC), and I acknowledge the fact that he/she is physically able to compete in camp activities. I hereby authorize Greg Guiler and/or the Director of Greg Guiler Basketball, LLC to act for me according to his best judgment in any emergency requiring medical attention. I acknowledge that I will be responsible for any cost (through family medical insurance or otherwise) incurred due to sickness or injury to my child. I do hereby, for & on behalf of myself and my heirs and legal representatives, release & forever discharge Greg Guiler Basketball, LLC, Greg Guiler, his Camps, Clinics, & Employees, and camp/clinic facilities from any & all claims, demands, actions or suits (hereafter, collectively “claims”) of every kind, nature and character, which I may have or may hereafter acquire, and for any and all damages, losses or injuries which may be suffered or sustained by my child in connection with my

• activities while on camp property. Moreover, any photographs or videos of my child may be used by Greg Guiler Baksetball, LLC & clinics/camps in any future promotional, advertising, or instructional materials. · Full payment is required with each registration. A 50% refund will be given for cancellation due to illness of any kind, including Covid19, or injury to registered participant before the event begins. Registration is at own financial risk as only a 50% refund will be given should the event have to cancel at any point due to Covid 19. All participants must have family medical insurance or be self-insured. I understand that by registering I agree to all the policies listed above. Additional Information and COVID-19 Waiver Note: Individuals are encouraged to adopt additional protocols consistent with their specific needs and circumstances to help protect the health and safety of all attendees. The virus that causes COVID-19 can be spread to others by infected persons who have few or no symptoms.

• . Even if an infected person is only mildly ill, the people they spread it to may become seriously ill or even die. Because of the hidden nature of this threat, everyone should rigorously follow the practices specified in these protocols, all of which facilitate a safe clinic. The virus that causes COVID-19 is still circulating in our communities. We should continue to observe practices that protect everyone, including those who are most vulnerable. Please note, public health guidance cannot anticipate every unique situation. Individuals should stay informed and take actions based on common sense and wise judgment that will protect the health of self and others. Pre-cautions required for athletes participating in Greg Guiler Basketball events in 2022 will be emailed with registration confirmation and are listed below..

• 1. Each participant must bring their own: a. Water bottle b. We will provide a basketball c. Mask (if they so desire) i. Masks are not required and could make it difficult to breath while participating in sports. Please use at your own discretion. 2. Before the clinic begins and at the end of the event participants must use anti-bacterial on their hands. 3. No additional guests a. Parents are asked to drop off at the South doors of building and are asked to check in their child at the registration table. b. Parents will be asked to pick up their child in carpool line style behind the building at the emergency exit doors (west side of building).

• 5. Before the start of the clinic each participant’s parent/guardian must screen their child/participant for the below. By signing the waiver, you confirm you have completed this task and your child/participant is able to participate. a. Check for a temperature lower than 99.6 Fahrenheit b. Confirm NO i. Shortness of breath or difficulty breathing ii. Chills iii. Repeated shaking with chills iv. Muscle pain v. Headache vi. Sore throat vii. Loss of taste or smell viii. Diarrhea ix. Feeling feverish c. No known close contact with a person who is lab confirmed to have COVID-19 6. Parents are required to notify Greg Guiler Basketball and participants in their group if their child/participant becomes ill, exhibits signs of Covid-19, is confirmed to have Covid-19, or comes in contact with a confirmed Covid -19 case.

• If this happens, we will contact those who have been within close contact to the infected individual. At the discretion of Greg Guiler Basketball, all further clinics may be canceled for up to 14 days. Only a 50% refund will be given for canceled events before the event begins. If event has already begun, no refund will be given.
