• I hereby authorize the directors of the Day to REACH Football Camp to act for me according to their best judgement in any emergency requiring medical attention. I also understand that due to the physical nature of the sport there is the possibility of injury. The REACH program, Chad Greenway's Lead the Way Foundation, or any entity involved with it assumes no liability for any injuries occurred throughout the camp.
