NEGTA 8U & 10U play request form

NEGTA waiver: I hereby acknowledge that I am familiar with all risks and hazards associated with participation, and I hereby assume all risks and hazards to such participation. I hereby release, absolve and agree to hold harmless Gainesville City, Gainesville Parks and Recreation, NEGTA board, sponsors, coaches, other participants and all other persons and legal entities acting on the behalf of NEGTA. I hereby authorize and consent to the use of the participant's visual image for appropriate purposes, including but not limited to: still photography, video, electronic and print publications, websites, and social media posts. I give this consent with no claim to payment. By completing this registration, I understand that I am agreeing to this waiver and all that it entails.

What level is the player?

• Beginner (just beginning match play)
• intermediate (has played JTT, school or tournaments)
• Never played tennis before

What age division will the player play?

• 8U (Must not turn 9 before August 31, 2023)
• 10U (Must not turn 11 before August 31, 2023)
• Too old for either group but is new to tennis
