Liability and Picture Policy

• I, as an adult or legal guardian of the minor child(ren) listed within this registration, have given my consent for myself/him/her/them to volunteer for Vacation Bible School at King of Kings Lutheran Church. I understand that there are risks involved in this event, and hereby release the Church, its staff, and volunteer workers from any and all liability for any injury, loss, or damage to person or property that may occur during the course of this event. In the event that myself or minor children listed within are injured and require medical attention, I consent to any reasonable medical treatment as deemed necessary.

• In the event treatment is required from a physician and/or hospital personnel designated by the church, I agree to hold such person free and harmless of any claims, demands, or suits for damages arising from the giving of such consent. I also acknowledge that I will be ultimately responsible for the cost of any medical care should the cost of that medical care not be reimbursed by the health insurance provider.

• Picture Policy: During the week of VBS, we will be taking pictures/video of the volunteers and children participating and could post said pictures/video on the King of Kings social media, during church service online, or displayed on bulletin boards. Pictures/video will be for the use of King of Kings Vacation Bible School program. Our VBS Opening and Closing could be live-streamed on Facebook, Youtube, and our church website. No names or ages will be associated with any posted picture or video.

• * I agree with the Liability and Picture policy of King of Kings Lutheran Church and its Vacation Bible School program.
