• In consideration of my or my child's participation in the Online Classes/Program at Radha Krishna Temple, Allen, I, the undersigned user, on behalf of myself, my heirs, representatives, executors, administrators, and assigns HEREBY DO AGREE TO RELEASE, INDEMNIFY, AND HOLD HARMLESS JKYog and Radha Krishna Temple (RKT), their officers, agents, volunteers, employees, and sponsors Jagadguru Kripaluji Yog (JKYog) from any cause of action, claims, or demands of any nature whatsoever, including but not limited to, a claim of negligence, which I, my heirs, representatives, executors, administrators, and assigns may now have, or have in the future against JKYog on account of personal injury, any direct or indirect loss or any damage to tangible or intangible property, data, goodwill or reputation, arising out of or in any way related to my or my child use of the online portal, whether that use is supervised or unsupervised, however the injury, loss or damage is caused, including, but not limited to the negligence,

• misconduct, act or omission on my or on my child's behalf by or during the use of the online portal provided by Radha Krishna Temple, Allen. I also give permission for photographs and videos taken in the Kids Classes/Program to be used for publicity purposes. I AGREE AND UNDERSTAND THAT THE FEE PAID FOR MY CHILD'S ENROLLMENT IS NON-REFUNDABLE. Registrations are first come first serve basis. JKYog and Radha Krishna Temple (RKT) have clarified that after they reach the number of registrations that they can accommodate, they will keep the entries on the waitlist and if they can not accommodate them,they will refund the amount. By submitting this form I am accepting these terms and conditions of JKYog and RKT, which shall be applicable to the enrollment of the classes/Program.

• I further agree and acknowledge that to the maximum extent permitted by applicable law, Jagadguru Kripaluji Yog (JKYog) and Radha Krishna Temple (RKT) of Dallas entities shall not be liable for any indirect, incidental, special, consequential or punitive damages, or any loss of profits or revenues, whether incurred directly or indirectly, or any loss of data, use, goodwill or other intangible losses, resulting from the participation in the program, or any unwanted intrusion into the video conference call by one or more individuals, hackers and/or internet trolls who may exploit or work around the video/audio application's security features, causing disruption to the services (also called as Zoombombing, Zoom-bombing, Zoom-attacks or Zoom raiding.
