Cancellation Policy
September 27, 2022 is the last day to cancel your registration. Request for refund will not be accepted after that date. All cancellations will be assessed a $25 cancellation fee. Contact ginavaldemar@coastal-vision.com by 9/27/2022 if you wish to cancel.

Health Acknowledgement
By attending Coastal Vision Medical Group's Continuing Education Event, you are confirming that you and all persons in your party: are not experiencing any of the following symptoms of COVID-19 identified by the Centers for Disease Control and Prevention at www.CDC.gov, including: 

• Fever or chills
• Cough
• Shortness of breath or difficulty breathing
• Fatigue Muscle or body aches
• Headache
• New loss of taste or smell
• Sore throat
• Congestion or runny nose
• Nausea or vomiting
• Diarrhea

Registrants who cannot confirm all of the above criteria must not attend Coastal Vision Medical Group's Continuing Education Event. If at any point during your attendance you do not meet the above criteria, you will be asked to leave and return home. If you need to cancel your registration, please contact us.

By attending the Coastal Vision Medical Group's Continuing Education Event, you acknowledge that you understand and will abide by the above health-and-safety requirements.
